Process Overview

Step 1

For initial evaluations, we recommend scheduling an initial zoom call that will also include an on-line neurocognitive test that can be taken prior to the discussion with one of our clinicians. Please complete the information form and an information email and testing link will be sent to you. You will also be allowed to schedule a time for the zoom call. Select the clinician discussion option under testing button below. There is a $45 fee that can be paid securely on-line.

Step 1-A

If you prefer an in-person appointment initially, you can schedule that as well. Simply choose the in office option. You will receive and email with schedule confirmation and on-line testing link. While we recommend taking the neurocognitive test in the comfort of your own home, the test can be administered in one of our facilities, (keep in mind that it is a 30-40 minute test followed by a 20-30 minute evaluation). There is a $135 fee that can be paid securely on-line at point of scheduling, (we can provide you with the necessary paperwork to be reimbursed by your insurance company if you desire).

Step 1-B

If you prefer to take the neurocognitive test only and have a telephone follow-up once it is complete, you certainly can do so by selecting the neurocognitive test option after clicking on testing below. There is a $20 fee for this service.

Step 2

In a quiet room, sit in front of your computer or tablet device, (we do not recommend using cell phone), click on the testing link. Please allow 30-40 minutes for testing. If this is your first test it will become your baseline. A baseline allows us to compare a future test with this first one. Parts of the test are timed but do not rush through. Go at your own pace.

Step 3

Your test results will be compared with other individuals your age and sex in order to get an idea of how you score against a similar average test taker.

The test will be scored and the results will be shared with you. We will also schedule time for you to speak with a clinician to explain the results. You will also be given your concussion score or MAC-Index.

Step 4

If the zoom call or in office appointment was done prior to the testing, we will schedule a clinician appointment in order to review the results and presents recommendations from our doctor to you, (this can be by zoom or in-office). We can also assist with finding a doctor near you if you wish for aftercare if you are not local. You may have the results forwarded to a physician of your choice should you elect to do so, (no additional cost). An explanation letter will accompany the results to your physician.
